Ontario Health Home and Community Care Support Services (formerly known as Local Health Integration Networks, or LHINs) and each local Health Service Provider (HSP) sign Multi-Sector Service Accountability Agreements (M-SAAs). The document outlines the services to be provided or performed over a multi-year time frame.

Ontario Health Home and Community Care Support Services represent several geographic areas within Ontario. Given the extended reach of our programs through Toronto, York Region and Simcoe County, LOFT has agreements with 3 areas. They are the Toronto Central, Central, and North Simcoe Muskoka.

Therefore, in compliance with these agreements, we have made copies of our M-SAAs available below. This is part of our commitment to transparency and to the quality of service we provide.

Why are M-SAAs important?

M-SAAs equip health service providers with funding for operating their programs and services. In addition, it creates a relationship between the government and health services providers. In sum, thanks to M-SAAs, Ontarians can have improved access to high-quality health care services.

What are Ontario Health Teams?

Ontario Health Teams (OHTs) provide a new way of organizing and delivering care that is more connected to patients in their local communities. Under Ontario Health Teams, health care providers (including hospitals, doctors and home and community care providers) work as one coordinated team – no matter where they provide care.
