Supported Housing Services – South Simcoe Hub
Supported House Services – South Simcoe Hub (SHS-SSH) offers support to individuals, 16 and older, living with mental health and/or addiction challenges. This program serves people living in Simcoe County Housing and Cooperative buildings located in Bradford, Beeton and Allison.
A mobile team of Case Managers and Support Workers work with individuals in their homes. Team members are experienced in supporting clients with mental health, addiction and psychogeriatric concerns.
SHS-SSH helps clients navigate the health care and social services systems, and coordinate the care and services each individual needs in order to achieve optimal health and well-being. In addition to these system navigation and service coordination services, Care Coordination is available for people with particular complex issues who require the services of multiple health care practitioners. The program also provides crisis intervention.
Clients who wish to do so can also receive education and support in maintaining their homes and assistance with activities of daily living, helping to ensure they are able to remain safe within their home.
