Seniors Assisted Living at Bradford House
Bradford House, provides supportive housing services to vulnerable and at-risk older
adults and seniors living with mental health and addiction issues as well as multiple physical health concerns.
The program seeks to serve people who, because of their care needs, require more frequent support and assistance that is available through home care, but who do not need the medical monitoring or supervision provided by a long-term care home.
Bradford House is located in a landmark, LOFT-owned building just a short walk from the historic downtown of Bradford, ON. The building itself as a venerable history, having been built as the home of an area doctor at the beginning of the 20th century. Since then it has been added to several times and now provides accommodation for 54 residents in single and double rooms.
Services Provided to Residents
Bradford House responds to each resident’s needs, with the goal of assisting clients to live safely ad in good health in the community as they age and to postpone for as long as possible, or eliminate entirely, the need for institutional care.
The programs provides a range of services and supports designed to enable older adults and seniors to live safely and make Bradford House their home. 24-hour on-site services are offered in response to each resident’s individual needs. A full meal program is included and support services are available for:
- Care coordination
- Intensive psychogeriatric case management
- Personal care and assistance with the activities of daily living
- Essential housekeeping, laundry, shopping and banking
- Medication support
- Escorts to appointments
- 24-hour emergency response
- Crisis intervention and support
- Assistance in learning or relearning basic life skills.
Psychogeriatric Case Management
LOFT Psychogeriatric Case Managers offer one-on-one support. They help clients navigate the health care and social services systems. Case Managers assist clients in developing individualized service plans in order to help them get the medical, financial, emotional, spiritual and recreational supports they need to be healthier, happier and as independent as possible.
Mental, physical and spiritual health are improved by social interaction and the sense of belonging that comes from a welcoming and supportive community.
Bradford House’s social recreation program offers residents opportunities to enjoy life, try new things, and become involved in their local community.
From summer barbecues to Holiday celebrations, we endeavour to respond to the interests of our residents.
The goal is to ensure that clients have access to interesting and stimulating activities and social opportunities though regular in-house events and group outings in the community.
