Community-Based Support for Seniors
LOFT Seniors Services has been a leader for more than 30 years, providing specialized, community-based housing and support services to vulnerable and at-risk seniors with complex challenges including mental or physical health issues, addictions, behavioural challenges, dementia, absence of family support, social isolation, cultural dislocation and poverty.
Many LOFT clients have faced the challenges of having no place where they can live with dignity and independence. At LOFT, we provide each client with the support he or she needs. We help our clients gain housing and remain independent in a home of their own for as long as possible, avoiding unnecessary hospitalizations or pre-mature moves into long-term care homes.
Research shows that we all remain happier and healthier the longer we are able to live in our own homes. We have seen for ourselves that when seniors have a home with appropriate support, there is a dramatic reduction in 911 calls, emergency department visits and hospitalizations. Our clients are seniors who are able to live independently with support, who do not need to be in a nursing home or a hospital, but for whom there are often few housing choices.
Our seniors’ programs encourage the development of communities of mutual support where everyone is welcome and no one is judged. Client communities provide activity and social interaction that significantly improve quality of life. There is also an added level of stability and security when people know their neighbours and look out for each other.
LOFT broke new ground in providing community-based supportive housing services for seniors with mental health and addiction challenges. The skills and knowledge developed from years of working with at-risk older clients gives us a breadth and depth of experience that is invaluable as we reach out to support more at-risk seniors in more ways.
Assisted Living for High Risk Seniors provides a range of services and supports designed to enable older adults and seniors to live safely in apartments of their own or in a boarding home environment. Services include assistance in navigating the social services and health care systems, connecting with professional health care providers, coordinating needed services and escorts to appointments. LOFT also provides assistance with the activities of daily living including help with personal hygiene, housekeeping and nutrition, medication reminders and assistance in learning or relearning basic life skills.

Psychogeriatric Case Management offers community-based one-on-one support and provides crisis intervention, system navigation and advocacy to link clients to needed services and resources. Our case managers assist clients in getting the medical, financial, emotional, and recreational supports they need to be healthier, happier and as independent as possible.
Behavioural Support Services are offered within the northern part of the City of Toronto, York Region and South Simcoe – the area served by the Central Local Health Integration Network or Central-LHIN. This program provides Mobile Support Teams to assist seniors living at home or in long-term care homes, who are experiencing responsive behaviours, or behavioural issues resulting from complex and challenging mental health, dementia or other neurological conditions. Community Re-Integration Services: Seniors who have been hospitalized often find themselves unable to be discharged, especially when there is no family support or appropriate housing available. If they were homeless prior to entering hospital, or have been in hospital for an extended period during which they have lost their housing, the challenge becomes even greater. LOFT offers specialized seniors’ transitional housing, including intensive, individualized support services, to enable seniors to safely and successfully return to living in the community.
