We accept referrals to the Integrated Psychogeriatric Outreach Program (IPOP) from medical doctors, health professionals and community agencies.

If you have your doctor’s support and approval, you can also self-refer by calling 1-844-798-6920 or by faxing referral forms to 1-844-834-4638.
