Crosslinks Street Outreach: York Region Homeless Network.
Crosslinks Street Outreach is the essential hub of a new service delivery model. Formed in October 2008, the York Region Homeless Network is a partnership embracing LOFT, the municipalities and regional social service providers, to form a comprehensive and coordinated network of services enabling a holistic approach to support for the homeless.
The Network brings together such services as community outreach, housing, medical and addiction services, financial, social and peer support, public health workers, and a team of mental health specialists.The goal is to provide each client with the fastest and easiest possible access to the support and services they need to help them take control of their lives, find housing, recover their health and begin their journey of recovery.
For over 10 years prior to the formation of the Network, the Crosslinks Street Outreach van has traveled more than 20,000 kilometres a year throughout York Region. As the region’s only street outreach service, the van makes contact with the region’s homeless and provided a critical link between them and the services they need.
